RM 10212.075: Required Evidence to Process a Name Change on the SSN based on a Certificate of Naturalization
Effective Dates: 05/23/2011 - Present TN 1 (02-10) RM 10212.075 Evidence Required to Process a Name Change on the SSN based on a Certificate of Naturalization Evidence of the name change event, the new name, and identity is required to process a name change based on Naturalization.
- A. Evidence of name change event
Refer to RM 10212.070 for acceptable evidence of a Certificate of Naturalization (N-550 or N-570) showing a new name as evidence of a name change event.
- B. Evidence of new name
- The new name shown on the Certificate of Naturalization is evidence of the new name.
- C. Evidence of number holder’s identity
Refer to RM 10212.015D and RM 10212.020 for evidence of identity requirements. If the Certificate of Naturalization does not show the prior name, but otherwise meets all requirements to be accepted as an identity document for a name change, accept one of the following documents in combination with the Certificate of Naturalization as one document and enter as a Certificate of Naturalization showing both the old and new name when entering as evidence of a name change in the Enumeration system. * the approved court petition for a name change which shows the prior name; or * the person’s birth certificate in the name as shown on a prior Numident record. NOTE: A person’s birth certificate establishes the prior name when it is shown on the prior Numident, since when a person requests to change the name to his or her birth name as part of Naturalization, a court petition may not be required. D. Example of acceptable evidence to process a name change based on a Certificate of Naturalization Clemencia Gonzalez requested the court to legally change her name to Carmen Gonzalez when she became a U.S. citizen. She applies for a name change on the SSN and submits evidence of the name change event, her Certificate of Naturalization showing her new name, Carmen Gonzalez. Carmen Gonzalez is not her birth name. The naturalization occurred one month prior. Carmen does not submit the approved court petition or birth certificate which would have shown her prior name, Clemencia, but she submits her State-issued driver’s license as evidence of identity in her prior name, Clemencia Gonzalez, which agrees with the name on her Numident record. We can process the name change.
