RM 10205: Social Security Number (SSN) Applications

An applicant must complete all required items on an application for a Social Security card to ensure the individual is assigned the correct SSN and to enable proper identification of the number holder in the future. However, in the event it is not possible to complete all items, process an application if minimum data requirements are met.

A. Minimum data requirements

The minimum data required for effective processing of an SSN application are:

  • Full legal name to be shown on card;

  • Name at birth and name shown on most recent SSN card, if different;

  • Citizenship or alien status;

  • Date of birth;

  • Place of birth;

  • Sex code (M or F);

  • Full name of one parent, preferably the mother's maiden name, if known (see 10205.145); and

  • Complete, valid mailing address.

B. When minimum data is not provided

Occasionally, an individual cannot provide the minimum data needed to complete an application for a Social Security card or cannot provide the evidence required to assign an SSN or to issue a replacement card, e.g., the applicant is homeless, has amnesia, or is mentally impaired. C. Evidence requirements

The applicant must provide the necessary documents to allow for the processing of the SSN application. For evidence requirements, see Evidence Required for an SSN Card, RM 10210.010 and RM 10210.015.
