DI 30005: Adjudicating Component Actions on Deficient Cases

A. Definition of an incorrect psychiatric review technique forms or residual functional capacity assessment form

A psychiatric review technique form (PRTF) or residual functional capacity (RFC) assessment form is incorrect when:

  • An SSA-2506-BK (Psychiatric Review Techniques), SSA-4734-BK (Physical Residual Functional Capacity Assessment), or SSA-4734-F4-SUP (Mental Residual Functional Capacity Assessment) has an incorrect finding or conclusion, and

  • A group I decisional deficiency, as defined in DI 30005.121C., does not exist.

B. Policy for correcting incorrect PRTF and RFC assessment forms

The review component may assume jurisdiction and prepare the PRTF or RFC assessment form if they have the capacity to do so, or they may return the case to the adjudicating component for correction. For a further explanation of corrective action policy, see GN 04440.238.

C. Adjudicating component procedure for correcting incorrect PRTF and RFC assessment forms

When the review component returns a determination with an incorrect RFC assessment for correction:

  • Prepare a corrected PRTF or RFC assessment form as requested on the SSA-847-U3 (SSA Request for Case Action). The original PRTF or RFC should remain in file as part of the audit trail. (The review component will annotate the original, “Do Not Process”.)

  • Place the corrected form in file.

  • Clear the case to the appropriate component.

  • Do not return the case to the review component unless they instruct you to do so.
